A cell therapy designed to fight blood cancers has driven severe, treatment-resistant rheumatoid arthritis (RA) into remission — another striking sign that cancer immunotherapy could be repurposed for autoimmune disease.
Researchers at Charité – Universitätsmedizin Berlin treated six patients (ages 31–69) with RA that had failed up to eight prior biologic or targeted therapies over a decade, using CD19 CAR-T — a patient’s own T cells, genetically engineered to hunt and destroy B cells carrying the CD19 marker. Those B cells drive the autoimmune attack in RA. The study was published in Nature Medicine.
The results
Disease activity dropped substantially in all six patients, and three achieved medication-free remission lasting up to a year. Autoantibody levels fell sharply, and inflammatory markers in the joints disappeared even months after treatment. “The identifying marker on many B cells — both abnormal B cells in cancers and disease-driving B cells in rheumatoid arthritis — is the surface molecule CD19,” explained Prof. David Simon, explaining the shared target.
Why it matters — and the caveats
The idea is a “deep reset” of the immune system: wipe out the misbehaving B cells and let a healthier population regrow. All patients had mild-to-moderate cytokine release syndrome (a known CAR-T side effect) that was manageable, with no severe neurological problems reported. But this is a very small study (six people), and CAR-T is complex and costly — so while the results are remarkable, much larger trials are needed before it could become a standard option.
