Losing weight is good for health — but a new study suggests the body’s immune system may not fully “forget” obesity for years. Obesity appears to leave a lasting molecular memory in immune cells.

Researchers at the University of Birmingham, in a roughly 10-year European study, found that obesity imprints durable epigenetic changes — chemical DNA methylation tags — on helper T cells (CD4+ lymphocytes) that can persist for 5 to 10 years after weight loss. Even after returning to a normal weight, people retained altered immune cells shaped by their earlier obese state. The findings were published in EMBO Reports.

What the marks do

The methylation changes affected two key biological pathways: autophagy (the cell’s waste-clearing process) and immune senescence (aging of the immune system). That persistent immune dysregulation could help explain why obesity-related risks — such as type 2 diabetes and certain cancers — can stay elevated even after weight normalizes. “Short-term weight loss may not immediately reduce the risk of some disease conditions associated with obesity,” said Professor Claudio Mauro.

Why it matters

The work reframes obesity as something that lingers biologically, not just on the scale — and suggests that sustained weight management over years, not a quick drop, may be needed to fully reverse the immune imprint. It also hints that future therapies might target these epigenetic marks directly. The study combined several human patient groups with mouse models; it’s mechanistic research, and how best to act on it clinically is still being worked out.
